DISCLAIMER RELEASE AND LIABILITY WAIVER

By participating in Qigong and Bioenergy class(es) & sessions, workshop(s) and/or meditation session(s)
private or in group, being present or remotely in long distance settings & over the internet, phone or other
means, I am fully aware that it is my responsibility to consult with physician prior to and regarding my
participation in Qigong and Bioenergy class(es) & sessions, workshop(s) and/or meditation session(s).

I represent and warrant that I am physically and mentally fit and have no medical condition that would prevent
me from fully participating in Qigong class(es) and Bioenergy session(s), workshop(s) and/or meditation
session(s) in person or remotely in long distance settings & over the internet, phone or other means.

I understand that Qigong class(es), Bioenergy session(s) and/or meditation sessions are not replacements for
professional medical care and that any information provided by the instructor is for educational purposes only
and is not meant as a substitute for medical advice.

I understand it is solely my responsibility to keep the instructor updated on any changes of my physical and
mental health and I understand that Erdan Bajrovic and/or Qi Heals You website, Qi Heals You Facebook page,
Qi Heals You Instagram page its affiliates and associates, representatives, agents and/or any other electronic
platform shall not be liable should I fail to do so.

In consideration of Qigong class(es), Bioenergy session(s) and/or meditation classes and/or sessions being
offered, I agree to assume full responsibility for any risks, injuries or damages, direct or indirect, known or
unknown, which I might incur as a result of participation in the services offered.

In consideration of being permitted to participate in Qigong class(es), Bioenergy session(s) and/or meditation
classes and/or session(s), I voluntarily and expressly waive any and all claims I may have against Erdan
Bajrovic and/or Qi Heals You website, Qi Heals You Facebook page, Qi Heals You Instagram page its affiliates
and associates, representatives and/or agents and/or any other electronic platform for injury or damages that I
may sustain as a result of participation in the services.

I, my heirs or legal representatives forever release waive, discharge and covenant not to sue Erdan Bajrovic
and/or Qi Heals You website, Qi Heals You Facebook page, Qi Heals You Instagram page its affiliates and
associates, representatives and/or agents and/or any other electronic platform for any and all injuries or death
caused by their negligence or other acts.

My presence in live or remote Qigong, and Bioenergy class(es), workshops and/or Meditation session(s)
constitutes express and irrevocable consent to the recording of my image, likeness and voice by Erdan Bajrovic
and/or Qi Heals You website, Qi Heals You Facebook page, Qi Heals You Instagram page and/or its affiliates
and associates, representatives, agents and/or any other electronic platform. I irrevocably grant them the right
to use my image, likeness and voice in social media, advertising and all other promotion capacities. I also
acknowledge and agree that my statement(s) and/or testimonial(s) be it verbal, written, audio or in video form in
private or in group setting can be used for promotional purposes in any manner without restriction of any kind.

I certify that I am at least 18 years of age (or if under 18 that I am joined by my parent of legal guardian) and
that I have read the above disclaimer release and liability waiver and fully understand its contents. I voluntarily
agree to the terms and conditions stated above.
